You will visit clients in their homes and provide initial assessments and reassessments of the physical, psychosocial, and environmental needs of clients and their families. You will develop appropriate and individual care plans, collaborating and embracing a multi-agency approach, including working with other healthcare professionals, and evaluate clients’ responses to care provided.

You may also act as the clinical care coordinator for assigned individuals/ families, initiating referrals as necessary, supervising and directing caregivers regarding the care of clients.
